APPLICATION FOR A PREMISES LICENCE (Section 17 or 71 of the Act)

Name of Applicant: EVA (UK) Ltd. Address of Premises: Lowerbarn Farm, London Road, Rayleigh, SS6 9ET.
Statement of relevant activities which it is proposed will be carried on, on or from the premises:
Provision of regulated entertainment, including films, indoor sporting events, live music, recorded music and performance of dance 9am to 1am daily.
Provision of late night refreshment from 11pm to 1am daily.
Sale by retail of alcohol from 9am to 1am daily.

The Licensing Register may be inspected at the Licensing Department, Rochford District Council, South Street, Rochford, Essex SS4 1BW between 9am and 4.30 pm (Monday to Friday except public holidays)

Any representations by a Responsible Authority or Interested Party must be made in writing to the above address by 2nd July 2015.

It is an offence knowingly or recklessly to make a false statement in connection with an application, punishable on summary conviction by a maximum fine of £5,000.
