A NURSE admitted that she did not conduct a ‘thorough’ risk profile the day before a Wickford mental health patient was found dead with 30 stab wounds.

Marion Michel, 56, of Jersey, died on March 4, 2022, while detained under the Mental Health Act 1983 as an inpatient at Brockfield House in Wickford, which is managed by Essex Partnership University NHS Foundation Trust (EPUT).

An inquest in Essex Coroner’s Office, Chelmsford, was heard by area coroner Sean Horstead.

Mojisola Adeji, a registered mental health nurse at EPUT, admitted to copying details from previous risk profile reports in February to new reports in March.

She said: “I have a usual template that I use from month to month.

“If there is no change in the patient’s behaviour I will use the same information from previous reports and add to the most recent risk profile.”

Ms Adeji later admitted this was wrong and not proper procedure.

She admitted to copying and pasting words from risk profile reports in November 2021 and adding them to February and March 2022 editions.

Marion was diagnosed with schizophrenia in 1998 and had a history of intense periods of mental illness when she would hurt herself and, on two occasions, her partner.

She was transferred to Brockfield House in 2018, after having been convicted of assault that year.

Previously, Dr Vivek Bisht noted that Marion was not suffering a psychotic episode in 2018 and there was an awareness by staff at Brockfield House that Marion could be violent when she was not having a psychotic episode.

It was also confirmed that Marion was most at risk of a violent episode when she was calm and quiet.

However, during nurse Adedeji’s risk profile of Marion on March 2, 2022, she stated she was looking for ‘aggressiveness’ and ‘mood swings’ when reviewing Marion.

This contradicted initial doctors and psychiatrist reports.

Furthermore, significant changes were to be flagged as factors that could increase risk.

Marion’s flatmate, who was identified in the inquest as Patient C, moved out shortly before Marion’s death.

Marion’s sister Karen spoke about how much this upset Marion, however this was not discussed during the risk profile.

The inquest is set to conclude on July 14.
